目的

探讨健康体检人群内脏脂肪面积(VFA)与新型炎症标志物中性粒细胞/高密度脂蛋白胆固醇比值(NHR)、单核细胞/高密度脂蛋白胆固醇比值(MHR)、中性粒细胞/淋巴细胞比值(NLR)的相关性。

方法

随机选取2020年6月至2022年6月于枣庄市立医院健康体检人群360人,根据VFA值分为内脏性肥胖(VO)组(VFA≥100 cm2)172人和对照组(VFA<100 cm2)188人,比较一般资料及生化指标;采用Spearman相关分析评估VFA与NHR、MHR、NLR的相关性,多元线性回归分析NHR、MHR、NLR的影响因素,采用ROC曲线评估NHR、MHR、NLR对内脏脂肪型肥胖的预测价值。

结果

与对照组比较,VO组BMI、VFA、皮下脂肪面积(SFA)、VFA/SFA(V/S)、甘油三酯(TG)、甘油三酯葡萄糖指数(TyG)水平升高,高密度脂蛋白胆固醇(HDL-C)降低(P<0.05);VO组人群NHR、MHR、NLR值升高(P<0.01)。在总人群中,男性的NHR、MHR值高于女性(P<0.05)。Spearman相关分析显示NHR、MHR均与性别(男1,女0)、总胆固醇(TC)呈负相关、与BMI、VFA、V/S、尿酸(UA)、TG、TyG呈正相关(P<0.05),NLR与V/S呈正相关,与TC、TyG呈负相关(P<0.05)。ROC曲线显示NHR、MHR、NLR三者预测内脏性肥胖的曲线下面积(AUC)分别为0.681、0679、0.624,最佳截断值分别为1.96、0.28、2.00。

结论

健康体检人群NHR、MHR、NLR与VFA、V/S相关,对内脏脂肪型肥胖有较好的预测价值。

Objective

To investigate the correlation between visceral fat area (VFA) and novel inflammatory markers neutrophil-to-high density lipoprotein cholesterol ratio (NHR), monocyte-to-high density lipoprotein cholesterol ratio (MHR), neutrophil-to-lymphocyte ratio (NLR) in healthy physical examination population.

Methods

A total of 360 people who underwent physical examination in Zaozhuang Municipal Hospital from June 2020 to June 2022 were randomly selected.According to the VFA value, 172 people were divided into visceral obesity (VO) group (VFA≥100 cm2) and 188 people were divided into control group (VFA<100 cm2), Compare general information and biochemical indicators; Spearman correlation analysis was used to evaluate the correlation between VFA and NHR, MHR and NLR, multiple linear regression was used to analyze the influencing factors of NHR, MHR and NLR, and ROC curve was used to evaluate the predictive value of NHR, MHR and NLR in visceral obesity.

Results

Compared with the control group, the proportion of male in VO group, BMI, VFA, SFA, VFA/SFA (V/S), triglyceride (TG), triglyceride glucose index (TyG) levels were increased, and high density lipoprotein cholesterol (HDL-C) was decreased (P<0.05); the NHR, MHR and NLR values of VO group were increased (P<0.01). In the total population, the NHR and MHR values of males were higher than those of females (P<0.05). Spearman correlation analysis showed that NHR and MHR were negatively correlated with gender (male 1, female 0) and total cholesterol (TC), and positively correlated with BMI, VFA, V/S, uric acid (UA), TG and TyG (P<0.05), NLR was positively correlated with V/S, and negatively correlated with TC and TyG (P<0.05). ROC curve showed that the area under the curve (AUC) of NHR, MHR and NLR for predicting visceral obesity were 0.681, 0679 and 0.624 respectively, and the best cutoff values were 1.96, 0.28 and 2.00 respectively.

Conclusions

NHR, MHR and NLR recorrelated with VFA and V/S in healthy physical examination population, and have a good predictive value for visceral fatty obesity.
